Support Coordination

Support Coordination assists participants of the National Disability Insurance Scheme (NDIS) to navigate the system and implement their plan by helping them to gain confidence and independence in managing all of their informal, community, mainstream and funded supports.

Support coordination can be provided at 3 different levels depending on what is funded in your NDIS plan:

  1. Basic level – Support Connection
  2. Medium level – Coordination of Supports
  3. High level – Specialist Support Coordination 

I’m an AHPRA registered Occupational Therapist experienced working in a case management role, I self-manage my own NDIS plan and have lived with a spinal cord injury in Darwin since 1999. I have extensive knowledge and understanding of the local community and support providers as both a consumer and health professional.

Fees

Fees vary depending on the service and are charged at an hourly rate. Charges apply to face-to-face service, preparation time, liaising with other providers, liaising with the NDIS participant/nominee, researching options, report writing and documentation. Some travel may be subject to fees depending on where the service is provided.

Please contact me for fee rates. Payments can be made via bank transfer upon invoicing or directly from your NDIS plan or plan manager.
